Thank you for choosing DiNinno Family Dental. We want your visit with us to be a pleasant one. Please help us to prepare to welcome you as quickly and efficiently as possible by completing the patient registration process online at least one day before your appointment.
Simply follow the online patient registration link to create your patient record. Your information will be submitted to us over a secure connection which will allow us to prepare your chart and verify your insurance benefits prior to your arrival. If you are unable to submit the record you can print it and bring it with you on the day of your appointment.

Depending on the services that you require, your initial appointment may consist of gathering of information relevant to your oral health. This may consist of an examination by the doctor and any radiographs, photographs and study models that are deemed necessary. Routine diagnoses and treatment plans can usually be given that same day. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your first visit:
- If you have had x-rays taken recently, i.e., within the last six months and are accurately labeled and of good quality, please bring them. If you prefer, our office staff can obtain these x-rays from your previous treating dentist. Please provide us with the necessary information.
- A photo ID, such as a driver’s license or student ID.
- A list of medications you are presently taking.
- If you have dental insurance, please bring the ID card or a listing of the insurance company name, claim office address and telephone number, as well as your personal ID number and your group number, if applicable.
